In the past, and still today in many places, when healthcare providers suspected or detected a reportable disease in a patient, a fax, phone call, or electronic form was used to get data to the public health agency, requiring manual effort and time away from clinical responsibilities. With the introduction of electronic case reporting (eCR), the necessary information is automatically extracted from the electronic health record (EHR) and routed to public health agencies, resulting in more timely and complete data, reducing the burden on busy health care providers, and allowing public health to respond more quickly to emergent issues.

National Picture: As of November 2023, there are 210 conditions that can be reported using eCR1. This includes a broad range of conditions such as birth defects, infectious diseases, chronic diseases, injuries, and more. Both public health agencies and healthcare providers benefit from the implementation of this technology. One of the most important benefits is the secure and rapid bi-directional data exchange capability between healthcare and public health, enabling state, tribal, local, and territorial (STLT) health departments to respond to both routine and emergency public health diseases and outbreak situations more effectively. Also, eCR affords the opportunity to automatically report many conditions that are diagnosed clinically and are not easily reportable via other electronic means, but that are important public health problems, such as birth defects, injuries, cancer, or occupational/environmentally caused diseases. Additionally, the reporting burden is removed from healthcare providers and the data entry burden is removed from public health, allowing more time for both parties to focus on understanding the health issues and taking actions to treat patients or prevent further illness.

Adoption of eCR technology accelerated greatly during the COVID-19 pandemic. Prior to the pandemic, there were 187 facilities sending eCR for five pilot conditions.2 By the end of November 2023, over 29,600 facilities were using this technology and uptake is projected to continue increasing (see https://www.cdc.gov/ecr/facilities-map.html).3 The Centers for Disease Control and Prevention (CDC) intends to continue expanding the breadth and depth of adoption of this technology and have integrated it into their Public Health Data Strategy(PHDS). One of the milestones of the PHDS is to have 38 jurisdictions ingesting eCR into their surveillance systems and that the percentage of critical access hospitals ending eCR increases to 35% by the end of 2024.4

Quotes: “This new process—electronic case reporting—is really just… revolutionizing the way that we can get reports from providers.
